How do I bill a missed visit?

Only if the cancellation policy says so. Reference the policy on the invoice line.

Are consumables taxable?

Usually the same as the service, but check local rules. The tax rate is set per line in the generator.

Can I add a deep clean?

Yes, as its own line with the date. It keeps the recurring price defensible.
